PAB Membership Nomination Form

This form should be completed by a representative of one of the following constituency groups:

  • Associated Students of UCM
  • Graduate Student Association
  • Academic Senate
  • Non-Senate Academic Council
  • Staff Assembly

Please complete this form to submit a Nominee from your constituency group to be a considered for membership on the Police Accountability Board (PAB). Important documents for you and the Nominee to review prior to submitting this form include:

Please review the PAB Code of Ethics and PAB Bylaws prior to submitting this nomination. Provide a brief statement of interest prepared by the Nominee. The statement of interest should describe what the Nominee will contribute to the Board and any related experience (work, volunteer, research, activism etc).

By submitting this nomination, I attest on behalf of my constituent group that the Nominee commits to and is available to serve on the PAB for a one-year term. I believe the Nominee will serve PAB impartially and abide by the ethical and professional standards of the PAB Code of Ethics in their review of cases of alleged police misconduct.
